Planet Name: Fairly simple; the name of your planet.
Planet Type: What type of planet is it? Temperate, Volcanic, Oceanic, Airid, Gas Giant, etc.
Star System: What type of Star System does your planet reside in? White Dwarf, Red Giant, etc.
Habitable: A simple yes or no question: Does your planet support life?
Galactic Coordinates: Where your star system (and therefore, your planet) is located on the Galaxy Map, in xx-yy coordinates.
Sentient Species: Whatever kind of sentient species resides on this planet, if applicable.
Notable Non-sentient Species: Any sort of interesting non-sentient species, like a large four-legged animal that is resistant to energy weapons.

Major Cities: Other major cities on the planet, again, if applicable.
Terrain type(s): A brief overview of the type(s) of terrain on your planet.
Points of Interest: Any other important places on your planet, like a unique geographic feature, or where a Stargate might be located.
History: The history of the planet. If the planet is fairly boring, like an unihabited desert planet, this neednt be over a paragraph long. An inhabited planet should have at least three or four paragraphs.
